If they are not related or as a friend, they can fall in love, and/or marry your look-alike, unless if you change it to something else. You can choose from a bigger variety of relationships of a certain Mii towards the real you. The Mii Maker has been optimized to make it a much more flexible and improved customization. They also act uncomfortable when nearby their allergic item. When the player discovers their allergy by giving it to them for the first time, they will always reject to eat, drink or use it. They will be allergic to one or two food(s), or sometimess, a collectable. This is randomly determined when a new Mii is created on the island. Miis can also take injuries during physical activity, in which the player can send them to the hospital by paying some money to help their wounds heal. More illnesses, such as the chicken pox, flu, ear infections, and fevers, are added. A Hospital’s added to the main island to help sick Miis. A rain cloud no longer rains over them, but instead an ill Mii is seen sitting around, crying, coughing/sneezing, or any other of their symptoms.
In Switch, this system is widely expanded for much more realistic illnesses. Illnesses: In the original Tomodachi Life game for 3DS, there were two illnesses: a cold and stomachache.In the Mii Maker, a Mii can be referred to as a he, she or they. Miis can also be transgender or non-binary. The player can set a Mii's sexuality via the Mii Maker, where they can be heterosexual, homosexual, or bisexual. This feature is turned off by default, but can be changed in the settings to the player’s liking. People of the same sex can fall in love with eachother and marry. LGBTQ+ relationships: As mentioned by Nintendo, now same-sex relationships are in the game.Miis can be seen feeding them, petting them, and doing all the things they need to clean up after them. They can own either at least one dog, cat, bird or rodent.

Each island is complete with its own set of values and exclusive events. Unlike the previous game, the player can choose from multiple designs for their island to suit their needs.
